Investigation Summary
At 4:15 p.m. on December 4, 2018, an employee was grading hides in the basement and began to walk up the basement ramp toward the yard. The employee was struck by the right rear tire of a powered industrial truck. The employee fell and was crushed by both right tires of a JLG Telehandler industrial truck Model 1055 (Serial No. 0160071520) that was backing up the ramp. The employee was killed.
