Investigation Summary
Atg 1:00 p.m. on May 16, 2017, an employee was using a tape measure to get a measurement inside of an unprotected excavation, measuring approximately 6.5 feet deep, when it collapsed. The employee was struck in the midsection by the collapsing excavation wall and became buried. The impact of the falling earth crushed the employee's internal organs, including rupturing his aorta. He suffered severe internal bleeding and was killed.
