Investigation Summary
At 5:15 p.m. on September 13, 2018, an employee was completing connections for a new sewer line and had to stop work due to an irritating substance that was used at a nearby Federal Corrections Center permeating the worksite. The employee waited approximately 45 minutes before going back to the work area to collect his tools and leaving the work site. The employee drove home and experienced shortness of breath and coughing. The employee's wife drove him to the local hospital, where he was diagnosed with having a heart attack. The employee died while at the hospital from blockage of the coronary blood vessel.
