Investigation Summary
At 1:30 a.m. on June 8, 2018, an employee was in a pit reconnecting a strip of scrap trim that was trimmed from the coiled sheet of steel and being collected on the scrap winder machine. The employee entered the pit with the machine operating. While in the machine, he told the operator to jog the machine to provide a longer strip to make the connection. The employee had part of his body between a structural beam and the oscillating guides of the scrap winder. When the employee instructed the operator to jog the machine, the operator did not have visual contact with the employee. The operator jogged the machine as instructed, and the employee was crushed between the beam and the oscillating rollers, receiving fatal wounds to the head and neck.
