Investigation Summary
At 12:45 p.m. on October 16, 2017, a 60 year old, male, truck driver was applying locks on a door of a refrigeration trailer. The employee was in the process of applying locks on the door of a refrigeration trailer. At the same time, a tractor/trailer that was across the parking lot began to roll away and ran into the employee and trailer, pinning the employee against the two trailers and causing fatal crushing injuries. The moving trailer did not have the brakes properly set which caused it to roll away from the building.
