At approximately 11:15 p.m. on September 20, 2020, an employee entered a walk-in refrigeration unit to pull orders from the rack shelf. The employee was alone in the work area. The employee was found facing down on the ground of the walk-in refrigeration unit. The employee sustained a head injury and was taken to the hospital and later died.
