Investigation Summary
At 8:00 a.m. on May 31, 2019, an employee and coworker were rigging down when they saw two rattlesnakes and were trying to kill them. The employee was lying across a gear box of pump jacks when the jack activated and struck the employee, sustaining three fractured ribs, a punctured right lung, a fractured right leg, and a ruptured liver. The employee later died at the hospital.
