Investigation Summary
At 10:20 a.m. on November 9, 2017, and employee was removing planks from a scaffold platform. The employee ran out of planks to stand on and stood on the 2 inch diameter pipe frame. The employee was not tied off with fall protection. When using both hands to pass a plank to another employee above, the employee lost his balance and fell from the 100 ft scaffold, resulting in death from multiple fractures.
