Investigation Summary
At 4:45 p.m. on July 26, 2017, an employee was standing on a pallet approximately 6 feet above ground trying to connect a fan to a hanging electrical cord. The employee grabbed onto a 480v electrical pendent and fell to the ground. The employee was electrocuted.
