At 1:00 p.m. on January 28, 2019, an employees was applying self-adhering base s heets (peel and stick) to a two-story steep slope roof, without fall protection. The employee slipped on a piece of the discarded self-adhering base sheet pape r backing, and fell off of the roof and was killed.
