Investigation Summary
At 8:30 p.m. on August 30, 2019, an employee was cleaning debris material from around the drag chain conveyor and tail pulley for the Trimmer Crossover Conveyor when his right arm was pulled into the machine. The employee amputated his arm below the elbow as well as sustaining a laceration to the neck. The employee was killed due to his injuries.
