Investigation Summary
At 8:45 a.m. on May 18, 2017, an employee was changing a cooling fan belt of a Toyota forklift (model number: 02-5PO35). During work, the vehicle fell off the lift it was elevated on and crushed the employee's upper torso. The employee was killed as a result of his injuries.
