Investigation Summary
At 7:30 a.m. on May 17, 2018, Employee #1 and a coworker climbed onto a wooden platform that was positioned on the forks of a JLG telehandler to be raised up to take pictures. As the JLG operator raised the employee and the coworker, the boom also extended. When the boom reached its maximum length, it jerked the platform, causing the employee to lose his balance and fall approximately 35 feet to the ground below. Employee #1 also struck the frame of the JLG telehandler, causing multiple broken bones and head trauma, killing him.
