Investigation Summary
At 2:15 p.m. on July 18, 2019, Employee #1 and two coworkers, employed by a construction company, were working in a wooded park. Employee #1 was driving a Yamaha Rhino 660 off-road utility vehicle on a dirt trail, and the three employees were clearing trees that had fallen on the trail. As Employee #1 drove up an incline, a tree limb protruding onto the trail pierced the front of the vehicle's firewall and struck Employee #1 in the stomach. Employee #1 was transported to the hospital and admitted for treatment of internal hemorrhaging and multiple injuries. He died July 19, 2019.
