Employee struck by or fall hazard and is killed

At approximately 4:00 p.m. on January 17, 2017, an employee was assigned his regular duties of cleaning around the crushing plant machine with a skid steer. At approximately 3:45 p.m., he departed the worksite as observed by a coworker and proceeded to a McDonalds approximately 0.8 mile away. There, the employee phoned emergency management services. He was transported to a local hospital and died shortly after. The employee died as a result of multiple internal injuries. How these injuries occurred could not be determined during the investigation. According to initial autopsy findings, the employee's injuries included, but are not limited to, multiple rib fractures, epicardial hemorrhages of the heart, lacerations of the left lung, diaphragm, right kidney, and spleen and abrasions of the right upper arm. A cause of death was not determined at the time this report is being submitted; however, according to the autopsy report, these injuries are likely associated with a crushing injury. It is possible that the employee was either struck by or fell but the cause of death was not unknown.
