Investigation Summary
At 8:30 a.m. on October 9, 2018, Employees #1 and #2 were standing on the bed of a tractor trailer after rigging a load of four structural steel beams being hoisted by crane to an upper level of a building under construction. Three of the four steel beams being hoisted became displaced and fell striking the both employees below. Employee #1 was hospitalized with multiple fractures. Employee #2 suffered multiple fractures and was killed.
