Investigation Summary
At 4:30 p.m. on August 22, 2019, Employee #1 told management that he was going to "dig out" around the sorter/screener machine. The employee left the area after speaking to management with a pitch fork in hand. At the end of the shift, management noticed that the sorter/screener machine was still running, and went to investigate. Employee #1 was found unresponsive, next to the conveyance system of the screener/sorter machine with blunt force trauma. He had sustained crushing injuries and died.
