Investigation Summary
At 7:15 a.m. on July 19, 2018, three employees were replacing an empty 55 gallon drum of paint reducer with a full drum of paint reducer. As the pump equipment was being transferred from the old empty barrel to the new full barrel, an unknown ignition source started a fire with the flammable vapors from one of the drums. After the initial flash fire, an explosion occurred. Five employees were hospitalized, three employees in the area were severely burned. Two employees died.
