Investigation Summary
At 3:15 p.m. on July 25, 2018, an employee was removing cement from a concrete machine mixer that employs a horizontal rod and paddles to assist in the mixing of cement with various finishes such as coloring, etc. The employee was using a pneumatic chisel (Husky) with an approximately 14 inch long bit for removing the dried concrete from mixer 2. While the employee was inside chipping away, the mixer came on and the employee was caught and wrapped around paddles and their rod resulting in his death. The employee sustained multiple traumas to upper body as well as internal injuries and died at the work location.
