Investigation Summary
At 11:00 p.m. on May 8, 2018, Employee #1 was working at an industrial laundry facility, running a Milnor wash line. After loading an industrial dryer by an automated shuttle conveyor, Employee #1 noticed a towel hanging from the dryer. As he removed the towel, the automated shuttle conveyor activated. Employee #1 was lifted up by the conveyor and became crushed between the conveyor and the industrial dryer, injuring his neck and chest. Employee #1 was killed as a result.
