Investigation Summary
At 9:02 a.m. on September 1, 2017, an employee was operating the D-Line Water Based Degreaser machine. The employee was degreasing 95 in. parts and, as the barrel turned over to dump the finished parts into a hopper, one or more parts fell out and prevented the hopper from moving. The employee went to clear the jam while there was another full barrel moving towards the unloading area. The employee was caught between the barrel and the hopper as the second barrel moved into position to be dumped. The employee was taken to the hospital where he succumbed to his injuries.
