Investigation Summary
At 7:30 p.m. on December 20, 2017, Employee #1 was cutting metal bands with large hand shears on bundled plywood weighing 2,000 pounds each. The bunks are bundled in two's (two bunks to a bundle) and are stood up on end as a shipping method. Employee #1 was to cut the bands and then use a forklift to lay the single bunks on the floor in a horizontal position. When the bands were cut one of the bunks fell and crushed Employee #1. Employee #1 was hospitalized with a broken arm and internal bleeding.
