Investigation Summary
At 10:15 a.m. on March 23, 2021, Employee #1 was walking towards a truck while t he truck was reversing from a parking spot in a dedicated parking area. The truc k had side mirrors with blind spot view, a rear view mirror, reverse lights, and a reverse alarm. The employee was aware that the truck was reversing as he was walking toward it; however, he made the decision to walk behind the truck when i t stopped momentarily and was struck and killed when it started reversing again.
