Investigation Summary
At 2:00 p.m. on September 11, 2017, an employee was on the 3rd floor balcony using a 5 foot A-frame ladder. The employee fell from the balcony and sustained severe head injuries. The employee was rushed to the hospital where he was subsequently pronounced dead.
