Investigation Summary
At 1:11 p.m. on May 15, 2024, Employee #1 was insulating glycol lines with foam and adding jacketing PVC. While performing these tasks, the employee fell and sustained fatal injuries. The employee was found unresponsive on the ground. Emergency services were called, but the employee was pronounced dead at the scene.
