Investigation Summary
At 10:30 a.m. on May 26, 2018, an employee was working at a soybean farm. He was monitoring the loading of soy beans into a semi-truck. He was standing on a Werner podium ladder. The semi-truck moved to distribute the soy beans evenly in the trailer. As the semi-truck was being repositioned, it came into contact with the ladder. The employee fell from the ladder, striking the back passenger portion of the truck tractor as he fell. He sustained blunt force trauma injuries to his chest and head. The narrative did not say whether he died at the time or later.
