Investigation Summary
At 11:45 a.m. on May 31, 2018, Employee #1, Coworker #1, and other crew members, employed by a utilities construction company, were installing a steel electrical pole. Employee #1 was spotting for Coworker #1, who was operating a forklift loaded with a pole. The pole fell from the forklift's tines and hit Employee #1's leg, causing blunt force trauma to the knee and leg. Employee #1 was transported to the hospital and admitted for treatment. He died as a result of his injuries.
