Investigation Summary

At approximately 9:00 a.m. on November 14, 2020, an employee was working at the 5-axis gantry cutting bed, machine Number 878-A, at Column GG46. A coworker found the employee between the machine and cooling tank wiping off metal shavings with blood on elbows and coming from the ear, coworker assisted the employee to a sitting position on the cooling tank, and contacted company EMS. The employee fell from approximately 26 inches to the concrete floor. The employee suffered a head injury and soft tissue injuries to both arms. The employee died from these injuries.
