Investigation Summary
On June 5, 2019, Employee #1, employed by a roofing company, was installing copper roofing to a 2.5-story single family residence. He left his work location to check a leak on the structure's back roof. Employee #1, who was not wearing fall protection, fell from the roof onto a concrete wall, a fall height of approximately 25 feet. He was killed.
