Investigation Summary
At 11:00 a.m. on March 18, 2019, an employee was preparing to strap a load of pipe to a truck. The load moved and broke the keeper stake. The employee had fallen trying to get out of the way of the pipe and the pipe struck the employee between the stomach and chest and rolled over the employee's head. The employee sustained blunt force trauma injuries and was killed.
