Employee is killed when caught in freezer

At 6:45 p.m. on April 30, 2022, an employee was in the blast freezer (Serial Number: 600, Model Number: BT43S3S, manufactured by: Advanced Equipment, Inc.), zone #2. The employee is required to go into the blast freezer to inspect the six freezer zones and determine whether to blast off any ice or snow accumulation in the zones. The employee was found by coworkers trapped in freezer zone #2 during a defrost cycle. By the time employees were able to disconnect the airlines to the freezer door and open it, the defrost cycle already affected the employee, who died of asphyxia.
