Investigation Summary
At approximately 3:30 p.m. on September 18, 2017, Employee #1 was installing a porch header beam. He was standing on and working from a single 2 in. by 12 ft. board. While installing the beam from the elevated work platform, he lost his balance and fell, striking the ground. Employee #1 suffered unspecified injuries in the fall and died.
