Investigation Summary
At 12:09 p.m. on March 28, 2018, an employee was removing a section of a safety gate/fencing to access a work area to add cable covers onto the cable trays around a lift frame of a Lodige vertical lift. The employee was positioned between the safety gate/fencing and the pathway of the counterweights of the Lodige vertical lift/hoist on the mezzanine level. The employee was struck by the counterweights which weighed 4,883 pounds. The employee sustained massive open head trauma and died.
