Investigation Summary
At 3:00 p.m. on January 4, 2022, an employee was standing on the trailer bed of a truck, placing a tarp over the truck's cargo. There were high winds noted during the time of the incident. The employee fell from the bed of the truck and hit his head on concrete. The tarp may have possibly caught a wind gust and thrown the employee off-balance. The employee suffered a head injury and was killed.
