Investigation Summary
At 9:30 a.m. on November 9, 2023, an employee was completing training on his third day. The employee was assigned to be the bulk truck driver. This training consisted of riding along with the current bulk truck driver to load, deliver, and unload feed. Just prior to the incident, the employee was tasked with loading the bulk truck. This process included climbing the ladder attached to the rear of the bulk truck in order to visually verify proper loading of the feed within each of the four compartments within the bulk truck tank. Without direct witnesses, it was speculated that the employee accessed the working platform on the rear of the truck and fell backwards to the ground below. The employee fell a distance of approximately 11 feet 9 inches to the concrete surface below. The employee struck the back of his head when contacting the ground. A co-worker and a supervisor saw the injured employee laying on the ground and rushed to his aid. Emergency services were called, and the employee was life-flighted to Lutheran hospital in Fort Wayne, IN. The employee was hospitalized from November 9, 2023, until dying from his injuries on November 15, 2023. The employee experienced blunt force trauma to the back of the head, directly leading to his death.
