Use this checklist as a tool to help you in your efforts to identify hazards in your facility and keep track of follow-up measures.

  1. Print out the checklist for all of the tasks in this area of your plant to take with you as you evaluate this area of your plant. To use this option, select "Print Blank Checklist" below and a new window/tab will appear with the checklist. Then, using your browser's print option, print the checklist.

  2. Use the online checklist below and make selections and comments before printing. To do this, first check the box next to each hazardous situation that you have identified. Then for each suggested solution, indicate whether the solution is currently in use, something you want to consider, or not applicable. Include comments about the solutions or applicable follow-up procedures. Note that you may wish to identify other solutions that would prevent or control the hazardous situation in your plant. At the end of each task, there is also a field for identifying other hazards related to the task or comments about the task.

    Select "Print Completed Checklist" and a new window/tab will appear with the checklist filled out. Using your browser's print option, print the checklist. The checklist printed with this option will include only the hazardous situations for which you have made entries. YOUR ENTRIES WILL NOT BE SAVED ON YOUR COMPUTER.

Forklift Operator
Collision Hazard

Use signs and policies to prohibit others from entering into these areas.

Use horn when appropriate to alert others.

Improve lighting where applicable.

Drive forklift backwards when moving loads.

Defective Forklift

Train operators to inspect and recognize potential problems or hazards.

Implement equipment inspection plan.

Report and correct problems as soon as identified.

Slips and Falls

Provide a roof or cover over forklift operations.

Reduce slippery surface conditions (e.g., snow removal).

Wear Anti-Skid Footwear.

Strains, Reaches, and Cuts

Provide gloves to pad and protect hands.

Provide ladders or platforms so employee can be repositioned to minimize awkward postures.

Minimize manual handling of large cages so employees are not exposed to conditions where strains may occur.

Locate controls for ease of use.

Maintain controls to ensure ease of operation.

Falls and Caught By Machinery

Follow good housekeeping procedures.

Properly guard machinery to keep employees away from moving parts.

Install guardrails around dangerous equipment to prevent injury.

Manual Back Dock Worker
Lifting, Bending, and Reaching

Install automatic dumper system.

Train workers how to lift properly to reduce injury.

Provide two persons to lift each cage.

Rotate to job with non-lifting task.

Disease Exposure

Provide back fan ventilation that moves dust and debris away from employees.

Provide workers with dust respirators and gloves.

Provide facilities for employees to clean up before eating.

Train workers to follow prescribed hand washing procedures.

Follow good housekeeping procedures.

Live Hang
Reaching To Conveyor and Shackles

Minimize forward reaches by moving shackle conveyor towards employee.

Minimize vertical distance between shackles and belt conveyor to reduce lifting of the arms to reach shackle and bending to reach supply conveyor.

Rotate employees up and down hanging line to vary the rate at which they perform the task.

Ramp shackle conveyor down toward the end of the line where there is less interference from birds on supply conveyor.

Provide height adjustable stands for shorter employees.

Provide anti-fatigue mats.

Disease Exposure

Install adequate ventilation systems that would reduce exposure to aerosolized dust. Supply make up air from behind employee and exhaust from far side of the conveyor.

Provide proper personal protective equipment and clothing.

Provide facilities for employees to clean up before eating.

Train workers to follow prescribed hand washing procedures.

Follow good housekeeping procedures.

Poor Lighting

Provide appropriate light for entry and exit.

Keep walkways clean and free of debris and obstacles.

Provide enough space between workers to avoid cuts.

Make sure all fans both for contamination control and comfort, that are mounted less than 7 feet above the working surface, are adequately guarded.

Standing For a Long Time

Install sit/stand stools, which allow employees to lean and have their weight supported while still remaining in an upright posture.

Install height-adjustable stands so cuts can be performed at mid-chest level.

Rotate to tasks that do not require prolonged standing.

Provide shoe insoles that cushion the feet and spread foot pressure over a larger surface.

Standing For a Long Time

Install sit/stand stools, which allow employees to lean and have their weight supported while still remaining in an upright posture.

Install height-adjustable stands so cuts can be performed at mid-chest level.

Rotate to tasks that do not require prolonged standing.

Provide shoe insoles that cushion the feet and spread foot pressure over a larger surface.

Falls, Back Injuries and Cuts

Provide effective drainage to prevent standing blood.

Provide rubber boots with high traction soles.

Sheath knives before entry to and exit from work areas.

Blood on Employee

Wear appropriate personal protective equipment (PPE), including mesh gloves, face shields, and protective clothing.

Develop and follow good housekeeping procedures.

Restricted Exit

Keep exits free and clear at all times.

Clearly and properly mark exits.

Implement emergency action plan.

Ergonomic Hazards From Use of Knives

Keep knives sharp and in good condition.

Remove damaged knives from service.

Use knives appropriate for the task.

Provide properly sized gloves.

Provide knives with a handle loop so they can remain in hand without the employee actively gripping the handle..

Electrical Shock

Train workers how to recognize electrical hazards.

Report and correct electrical hazards as soon as they are discovered.

Schedule preventative maintenance to inspect for frayed or damaged equipment.

Use appropriate equipment for wet locations.

Strangulation or Amputation

Properly guard and regularly maintain machines and equipment.

Use Lockout/Tagout program.

Use appropriate equipment for wet locations.

Noise

Provide workers with hearing protection devices and require their use.

Use belt-driven plucking machines rather than chain-driven machines.

Paw Room Grader
Standing For a Long Time

Install sit/stand stools, which allow employees to lean and have their weight supported while still remaining in an upright position.

Rotate to tasks that do not require prolonged standing.

Provide shoe insoles that cushion the feet and spread foot pressure over a larger surface.

Provide a foot rest in front of employees so they can shift their position.

Prolonged Bending of Neck

Conveyor should be at about elbow height of employee. Provide height-adjustable stands to properly position employees.

Tilt conveyor toward employee.

Rotate to other jobs that provide more movement.

Provide time and training about stretching to relieve neck fatigue.
