Investigation Summary
At 10:15 p.m. on January 12, 2018, an employee was cleaning some cheese scraps from an Ultrasonic Cutter No. 4 discharge tray transfer conveyor. The employee opened a side access door and inserted a 19.5 inch long white plastic stick with her left hand to dislodge some cheese slices. The employee had her gloved fingers that were holding the stick pulled into a nip point and she amputated her fingertips. The employee was hospitalized.
