Investigation Summary
At 8:30 A.M. on June 9, 2017, Employee #1 was working on the single trim saw line cutting 2 by 4 boards. The blade had a jam and was in a bind. Employee #1 cut off the saw and walked around to the side of the machine and reached in toward the blade to clean out the debris before the wheel stopped. The employee suffered severe laceration to 3 fingers of left hand and amputation of left pinky finger.
