Investigation Summary
At approximately 3:15 p.m. on October 30, 2012, Employee #1, Employee #2, and Coworker #1, were working in an oven filled with hot aluminum parts. The oven, a walk-in tunnel style, is used to treat aluminum extrusions. The oven is 83 feet long by 7.5 feet wide by 7.5 feet high. Workers load and unload the oven manually by pushing the racks of aluminum into and out of the oven on a roller conveyor system that is bolted to the floor. The racks are double stacked, with the top rack measuring 18.3 feet long, 2.7 feet wide and 2.9 feet tall and the bottom rack measuring 24.5 feet long, 2.7 feet wide, and 2.5 feet tall. The approximate weight of the combined racks is between 4 to 5 thousand pounds. After the oven's heat cycle is completed, the doors of the oven open at either side of the tunnel at set intervals. The oven temperature was approximately 200 degrees Fahrenheit. Workers wear hot mill gloves, safety glasses, and steel toed boots and work together in teams of two to clear out the oven. Workers are trained to push racks from the front position because there are no lights in the oven and the employee can observe how the bottom rack is contacting the flanged rollers from this location. Employee #1 was stationed at the front of the racks pushing on the first or second post on the right side facing the oven exit. Employee #2 was at the back post of the racks on the right side facing the oven exit. Coworker #1 was directly behind the racks facing the oven exit. Shortly after Coworker #1 contacted the rack, the racks came off the rollers and fell pinning Employee #1 and Employee #2 to the far oven wall. Employee #1 was pinned towards the middle of the racks and died. Employee #2 was pinned towards the end of the racks and was hospitalized for several weeks with severe burn injuries.
