29 CFR 1910.147(d): The established procedure for the application of energy control (the energy control procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about October 26, 2012 and on October 30, 2012, the employer failed to ensure that Oven #1 was locked out, including but not limited to locking out the electrical system and the gas line, before employees performed servicing and maintenance operations such as, but not limited to, unjamming racks loaded with aluminum product and removing aluminum product from the oven out of racks that cannot be moved.
