Investigation Summary
Employee # 1 was working the Derrick (Monkey Board) of a gas well rig when the well exploded blowing Employee # 1 off of the Monkey Board. Employee #1 was left hanging by his safety harness and lanyard line. Coworkers said Employee # 1 hung by his safety line for approximately 10 seconds to 20 seconds in the flames from the burning rig until his lanyard line melted from the heat. Employee # 1 fell approximately 54 ft to the floor below. Rescue units were called from the nearest local town, which is approximately 20 miles from the drill site. Police, the Coroner and the rescue unit all arrived approximately at the same time and Employee # 1 was transported to the local hospital approximately 40 miles from the scene of the accident. Employee #1 died.
