Investigation Summary
At 4:00 a.m. on November 15, 2014, five employees were attempting to unblock a plug in some piping at a facility's Crop Production Unit when a sudden release of Methyl Mercaptain occurred. Four employees died from unspecified injuries during the incident and the last employee was admitted to an area hospital where he was released the next day.
