29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. On or about November 15, 2014 and at the DuPont facility in La Porte, Texas, the employer did not implement its change of design (COD) procedure to manage changes when: a) Did not follow the ventilation fan operating procedure to restrict access to the Lannate/API building when the dry end ventilation fan was not operating. b) Did not follow the ventilation fan operating procedure to restrict access to the Lannate/API building when the dry end ventilation fan was not operating.
