At 7:00 p.m. on May 14, 2018, an employee was loading a trailer with a corrugated pipe while using a Caterpillar Model DP70N1 Forklift Truck and had secured a load on the truck's tongs before backing up. The employee started to back up without looking and struck a parked forklift that was being unloaded by hand. The employee was struck in the chest by the second forklift's prongs when they came through the employee's cab. The employee was hospitalized with a crushed chest.
