29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about May 14, 2018, the employer was unable to demonstrate that the use of a fabricated steel attachment to a Caterpillar fork truck (Model #DP70N1) did not affect the safe operation of the forklift. (The attachment was 2,800 to 3,000 pound steel 2-prong design that measured 23.9 feet long, 4.1 feet in height, and 5.4 feet wide.
