Investigation Summary
Employee #1 was using a carbon rod to air arc (cut) a large 1,200 to 1,300 lb piece of 5 ft 3 1/2 in. long by 15 1/4 in. wide by 5 1/4 in. high steel that was welded onto the side of a 15-ton steel ladle. It was to be removed because the company had a contract to recondition this ladle. The height was 5 ft at the low end and 5 ft 6 in. on the side to be rebuilt. A lifting hook had been welded onto the piece by another welder, as requested by his supervisor. Employee #1 did not hook up the piece being cut to the lifting lug, and continued to cut the piece off the ladle. The piece fell, crushing him against the concrete floor. Employee #1 was killed.
