Two employees are killed and a third sustains burns from a f

At 12:00 p.m. on August 12, 2012, three employees were working for a demolition contractor. They were demolishing a single-story building some 7.6 meters (twenty-five feet) tall. Two of the employees were using a shovel to clean blast material from the top of a deflector, when a third employee used a cutting torch to cut the bracket that held the deflector to the ceiling. When the employee used the cutting torch to cut the deflector from the ceiling, a flash fire occurred. OSHA sampling of the blast material in the blast room determined that the material involved was combustible dust. Two employees sustained thermal and possible blunt force trauma and they were killed. The third employee sustained burns and he was hospitalized.
