Investigation Summary
On March 18, 2011, Employee #1, a heavy equipment mechanic, was driving a loader with a pallet fork attachment to an orange grove to lift a piece of equipment. While traveling down the public roadway, the fork attachment came off (or was lowered) and was run over by the loader. That action and a resulting flat tire caused Employee #1 to be ejected in front of the loader. He was run over and killed.
