Investigation Summary
On November 20, 2010, Employee #1 was hand lighting a commercial fireworks display. As fireworks launched, just before the finale, Employee #1 was struck in the face with a four-inch shell as it exited a mortar. He was killed as a result of severe head and face injuries.
