Employee Is Killed When Struck by Lifting Mechanism

On September 18, 2008, Employee #1 was standing within 20 ft of a piece of equipment identified as an air winch. The air winch consists of two 11,000 pound winches mounted on a metal platform where a coworker sits and operates the controls. The air winch was being used to pull a concrete beam measuring 120 ft long by 10 ft wide by 8 ft tall. Seven beams had previously been pulled using this same Air Winch. The Air Winch platform was mounted to the concrete using four 2.5-in. bolts screwed into red dot anchors. Two chains were run from an anchor point on the dock, through an opening in the platform and latched back onto each chain. Approximately halfway through the pull the two chains broke at two links. The platform rose in the air and moved approximately 20 ft striking Employee #1. His head struck the pavement and his left leg was pinned under the platform as the platform came to rest. The coworker was thrown 6 to 8 ft in the air and struck a guardrail as he fell to the concrete dock suffering a sprained back. The coworker was treated and released. The coworker's injuries were not recorded. Employee #1's left leg was amputated just above the knee and he remained on life support until he died two days later.
